Summary

The AMYPAD trials successfully harmonized amyloid positron emission tomography (PET) scans across multiple centers, achieving high concordance for Alzheimer's disease diagnosis. This framework is now the EARL accreditation standard for multicenter brain PET scans.

Area of Science:

  • Neuroimaging
  • Alzheimer's Disease Research
  • Medical Diagnostics

Background:

  • Amyloid-PET imaging is crucial for Alzheimer's disease management.
  • The Centiloid method standardizes amyloid-PET measurements across different tracers.
  • AMYPAD trials aimed to validate robust amyloid-PET quantification in clinical settings.

Purpose of the Study:

  • To report a comprehensive approach to amyloid-PET implementation, including scan acquisition, harmonization, data storage, and sharing.
  • To validate robust amyloid-PET quantification in clinical settings within the AMYPAD trials.
  • To compare different Centiloid quantification workflows.

Main Methods:

  • Data from 28 scanners were harmonized using Hoffman phantoms.
  • Three quantification workflows were compared: AmyPype (PET-only), IXICO-LEAP, and the standard Centiloid pipeline (MRI-based).
  • Gaussian Mixture Modeling was used to assess Centiloid distributions.

Main Results:

  • Binary quantification showed excellent concordance (≥96%) between pipelines and compared favorably to visual reads (>90%).
  • Centiloid distributions in the DPMS and PNHS trials aligned with recruitment strategies.
  • The harmonization framework demonstrated strong cross-method correlations.

Conclusions:

  • The AMYPAD harmonization framework provides a practical approach for multicenter cohorts.
  • The framework has been adopted by EARL as the Brain PET/CT accreditation standard.
  • This standardization enhances the reliability of amyloid-PET quantification in clinical practice.
