Insomnia symptoms in older adults at high risk for Alzheimer's disease were surprisingly linked to better memory and attention. This suggests sleep disruption may not always negatively impact cognition in this population.

Purpose of the Study:

  • To determine if insomnia is associated with cognitive changes over time in a high-risk population.
  • To compare cognitive performance based on self-reported and objectively measured insomnia symptoms.

Main Methods:

  • Studied 348 dementia-free older adults, assessing self-reported insomnia using the Pittsburgh Sleep Quality Index (PSQI).
  • Objective sleep was measured via 7-day actigraphy in a subsample.
  • Cognition was assessed yearly for an average of 7.15 years, including memory and attention measures (RBANS).

Main Results:

  • Higher self-reported insomnia symptoms correlated with better baseline immediate and delayed memory.
  • Objective insomnia measures via actigraphy showed higher longitudinal delayed memory scores.
  • Individuals with objectively measured insomnia only had the highest longitudinal delayed memory scores.

Conclusions:

  • Counterintuitively, increased insomnia symptoms (both self-reported and objective) were associated with better cognitive function in individuals at elevated AD risk.
  • This association may indicate underlying protective lifestyle factors that enhance cognition but disrupt sleep.
