The evidence supporting STOPP/START version 3 (SS.v3) criteria was evaluated. Most START criteria have strong evidence, but fewer than half of STOPP criteria require reassessment due to limited high-level evidence.
Area of Science:
- Geriatric Pharmacology
- Evidence-Based Medicine
- Clinical Criteria Development
Background:
- The evidence base for STOPP/START version 3 (SS.v3) criteria is controversial, potentially hindering clinical adoption.
- This study critically appraises the references supporting the SS.v3 criteria.
Purpose of the Study:
- To evaluate the quality and appropriateness of references cited for the 190 SS.v3 criteria.
- To determine the level of evidence supporting each SS.v3 criterion.
Main Methods:
- All 454 references for the 190 SS.v3 criteria were reviewed by geriatric specialists.
- Evidence levels were classified (I-V) based on study design (RCTs, cohort, case-control, etc.).
- References were deemed inappropriate if mismatched, lacking support, or clinically discordant.
Main Results:
- A significant proportion of references (322/454) were reviews (70% STOPP, 71% START).
- High-level evidence (Level I) supported 43% of STOPP criteria and 88% of START criteria.
- 10 criteria (8% of STOPP) lacked appropriate supporting references.
Conclusions:
- Most START criteria are supported by robust evidence, unlike many STOPP criteria.
- A reassessment of STOPP criteria and their supporting literature is necessary.
- Clinicians must consider evidence limitations during shared decision-making with older patients.

Decision Making: P-value Method
First, a specific claim about the population parameter is proposed. The claim is based on the research question and is stated in a simple form. Further, an opposing statement to the claim is also stated. These statements can act as null and alternative hypotheses: a null hypothesis would be a neutral statement while the alternative hypothesis can...
