Summary

A pro-inflammatory diet is linked to reduced fertility in women without prior infertility issues. This dietary pattern score, EDIP-SGW, was validated and associated with lower conception rates, particularly in women with low insulin resistance.

Purpose of the Study:

  • Develop and validate an empirically derived dietary inflammatory pattern score for Singaporean women (EDIP-SGW).
  • Examine the association between the EDIP-SGW score and fecundability in women from the S-PRESTO study.

Main Methods:

  • Calculated EDIP-SGW score using food frequency questionnaire data and reduced rank regression, predicting hsCRP and GlycA.
  • Validated the score in the GUSTO study.
  • Estimated fecundability using discrete-time proportional hazards models adjusted for covariates (n=890), exploring interaction with insulin resistance (HOMA-IR).

Main Results:

  • The EDIP-SGW score effectively predicted inflammatory markers (hsCRP, GlycA) in both S-PRESTO and GUSTO cohorts.
  • Higher EDIP-SGW scores were associated with significantly lower fecundability (34% lower in the highest quartile; 13% lower per SD increase).
  • A significant interaction with insulin resistance was observed: pro-inflammatory diets reduced fecundability in women with low HOMA-IR but not in those with high HOMA-IR.

Conclusions:

  • A pro-inflammatory diet is associated with reduced fecundability in multi-ethnic Asian women without known infertility.
  • Insulin resistance may play a role in modulating the relationship between diet and fertility, warranting further investigation.
