Summary

Peripheral neuropathies (PNP) can cause chronic itch. Patients with PNP and itch show reduced endogenous itch inhibition, suggesting this may be a key mechanism driving their symptoms.

Purpose of the Study:

  • To investigate inhibitory pruritus mechanisms in patients diagnosed with peripheral neuropathies.
  • To compare conditioned itch modulation (CIM) and conditioned pain modulation (CPM) between patients with PNP and pruritus, patients with PNP without pruritus, and healthy controls.

  • Phenotyping 39 participants (16 PNP with pruritus, 8 PNP without pruritus, 15 controls) using quantitative sensory testing and nerve fiber density assessment.
  • Employing a novel conditioned itch modulation (CIM) protocol with electrically induced itch and a cold-water bath as a conditioning stimulus.
  • Measuring itch, pain intensity, and desire to scratch before, during, and after conditioning stimulus application.

  • Electrically induced itch was successfully generated and modulated by cold stimuli in all groups.
  • Patients with PNP and pruritus (PNPPRU) exhibited significantly reduced itch inhibition (CIM effects) compared to controls and PNP patients without pruritus (PNPNO-PRU).
  • Conditioned pain modulation (CPM) effects did not differ significantly between the groups.

Conclusions:

  • Reduced endogenous itch inhibition is a significant finding in patients experiencing chronic pruritus associated with peripheral neuropathy.
  • These results suggest that diminished descending itch inhibition may play a crucial role in the development and persistence of chronic itch in PNP.
