Cerebral Venous Sinus Thrombosis Following Neuraxial Labor Analgesia: A Case Report

Abstract:
We describe a case of cerebral venous sinus thrombosis (CVST) in a 25-year-old primigravid woman after receiving epidural analgesia during labor. Her initial presentation mimicked postdural puncture headache (PDPH), but her condition evolved to include progressive neurological symptoms. Neuroimaging demonstrated extensive thrombosis of the cerebral venous sinuses with associated subdural hematomas. Prompt endovascular thrombectomy combined with therapeutic anticoagulation achieved full neurological recovery. This report highlights the diagnostic challenges of differentiating benign postpartum headaches from potentially life-threatening etiologies and underscores how pregnancy-related hypercoagulability and neuraxial procedures may interact to precipitate CVST.
