Summary

Combining intense pulsed light (IPL) with CO2 fractional laser therapy offers a safe and effective treatment for post-traumatic hyperpigmentation (PTH). This combination therapy demonstrated superior results compared to individual treatments or no treatment.

Background:

  • Post-traumatic hyperpigmentation (PTH) is a common cosmetic concern following skin injury.
  • Existing treatments like intense pulsed light (IPL) and fractional CO2 lasers have limitations for PTH.
  • Evidence for the combined efficacy of IPL and fractional CO2 laser for PTH is limited.

Purpose of the Study:

  • To evaluate the efficacy and safety of combining IPL with fractional CO2 laser therapy for post-traumatic hyperpigmentation.
  • To compare the combined therapy against monotherapy (IPL or CO2 alone) and an untreated control group.

Main Methods:

  • Retrospective study of 48 patients with 66 PTH lesions.
  • Four groups: IPL + CO2 combination, IPL alone, fractional CO2 alone, and control.
  • Clinical improvement assessed using the Global Aesthetic Improvement Scale (GAIS) by independent physicians.

Main Results:

  • The IPL + CO2 combination group showed the greatest improvement, with 95.2% achieving marked improvement (GAIS ≤ 2).
  • 100% of lesions in the IPL and IPL + CO2 groups showed overall improvement (GAIS ≤ 3).

Conclusions:

  • Combined IPL and fractional CO2 laser therapy is a safe and effective treatment for post-traumatic hyperpigmentation in Asian patients.
  • Further research with larger cohorts and longer follow-up is recommended.
  • Optimization of treatment protocols based on pigmentation duration and skin phototype is warranted.
