Lower urinary tract symptoms in men with chronic low back pain: A cross-sectional study

This study aimed to assess the prevalence and severity of lower urinary tract symptoms (LUTS) in male patients with chronic low back pain (CLBP), and to explore the associated factors.

We conducted a cross-sectional study including men with CLBP. CLBP-related disability was assessed using the Oswestry Disability Index (ODI) and Lumbar Stiffness Disability Index (LSDI), anxiety and depression symptoms were evaluated using the Hospital Anxiety and Depression Scale (HADS), and sleep quality with the Pittsburgh Sleep Quality Index (PSQI). LUTS severity was evaluated using the International Prostate Symptom Score (IPSS) and its Quality of Life (QoL) question, respectively. Erectile function was assessed with the five-item International Index of Erectile Function (IIEF-5).

A total of 101 men with CLBP were included. The mean age was 52.14±12.89 years. Moderate-to-severe LUTS (IPSS>7) were present in 66.3%. LUTS-related QoL dissatisfaction (score>2) was reported in 26.7%. Erectile dysfunction (ED) was found in 83% (mean IIEF-5 score: 18.9±6). In univariate analysis, LUTS were significantly associated with age (P=0.04), absenteeism (P=0.03), pain intensity (P=0.05), anxiety (P=0.006), depression (P=0.003), and ED (P<0.001). Multivariate logistic regression identified anxiety and ED as independent predictors of LUTS.

LUTS were frequently reported in men with CLBP and were significantly associated with anxiety and ED.
