Purpose of the Study:

  • To investigate the causes, clinical characteristics, and outcomes of unplanned readmissions in patients with advanced cancer.
  • To identify factors associated with early unplanned hospital use post-discharge.

Main Methods:

  • Retrospective, multicenter study of 542 patients with advanced solid tumors in South Korea (2019).
  • Readmissions within 1 month of prior hospitalization were categorized: cancer progression, treatment-related, or other.
  • Analysis of unplanned hospital use within 1 month post-discharge for survivors.

Main Results:

  • Cancer progression (42.6%) was the primary cause of readmission, associated with longer hospital stays (median 12 days) and higher mortality (23.4%).
  • Treatment-related readmissions (37.3%) had shorter stays (8 days) and lower mortality (8.4%).
  • Among survivors, 24.9% experienced additional unplanned hospital visits within 1 month; home discharge was a significant predictor (aOR 4.82 for readmissions).

Conclusions:

  • Cancer progression is the leading driver of unplanned readmissions, correlating with worse outcomes.
  • Home discharge is a critical risk factor for subsequent unplanned hospitalizations, necessitating enhanced post-discharge monitoring.
  • Improved care coordination and monitoring are crucial for reducing readmissions and improving outcomes in advanced cancer patients.
