Abstract:
The geriatrician is interested in elderly people with a complex state of frailty or a complex, multipathological condition, proposing preventive, curative or palliatives measures. In this article, we describe their possible interventions in the care pathway for elderly subjects with so-called pathological or usual aging with identification and interventions targeted at frailty in the context of home care, where most people wish to live despite old age or dependency.
