    Summary

    Chenodeoxycholic acid (CDCA) is an oral treatment for gallstones, but animal studies raised concerns about liver toxicity. This study found no evidence of hepatotoxicity in patients receiving CDCA, indicating its safety in human treatment.

    Background:

    • Oral chenodeoxycholic acid (CDCA) is used to dissolve gallstones.
    • Recent animal studies suggest potential hepatotoxicity of CDCA.

    Purpose of the Study:

    • To evaluate the hepatotoxic effects of CDCA in patients with gallstones.
    • To assess liver function and histology during CDCA treatment.

    Main Methods:

    • Liver function tests (serum bilirubin, albumin, enzymes) were monitored monthly for six months in 25 patients treated with CDCA.
    • Bromosulphthalein (BSP) clearance and transport maximum were assessed.
    • Liver biopsies were performed on patients receiving CDCA, those with untreated gallstones, and those previously treated with CDCA.

    • No significant changes were observed in liver function tests or BSP kinetics during CDCA therapy.
    • Fasting serum bile acid concentrations were elevated but within expected ranges.

    • CDCA, at doses of 0.5-1.5 g/day, does not appear to be hepatotoxic in humans.
    • The study supports the safety of CDCA for gallstone dissolution in suitable patients.
