Background:

  • Right atrial appendage aneurysm, also known as giant right atrial appendage, is an exceedingly rare condition.
  • Few cases have been documented in scientific literature, necessitating a comprehensive review.

Purpose of the Study:

  • To systematically review published cases of right atrial appendage aneurysm.
  • To analyze patient demographics, clinical presentation, diagnostic methods, and outcomes.

Main Methods:

  • An extensive electronic search was conducted for case reports and series published up to March 2025.
  • Clinical data including age, sex, symptoms, electrocardiography, imaging findings, and treatment outcomes were extracted and analyzed.

Main Results:

  • Forty-four cases were identified, predominantly affecting males (68.2%) and presenting in the third decade of life.
  • Common symptoms included palpitations (27.3%) and dyspnea (18.2%), though 40.9% were asymptomatic.
  • Echocardiography (93.2%) and CT/MRI (79.5%) were key diagnostic tools, revealing a mean aneurysm size of 93 × 70 mm. Congenital heart abnormalities, such as atrial septal defects, were present in 27.3% of patients.
  • Surgical treatment was employed in 50.0% of cases, with 47.8% managed medically. One case showed aneurysm size reduction after atrial septal defect closure, and one death (2.3%) was reported.

Conclusions:

  • Right atrial appendage aneurysm, despite its rarity, is associated with significant morbidity.
  • Symptomatic patients benefit from surgical intervention for aneurysm removal.
