Summary

Adding infliximab to glucocorticoids improved renal response time in acute interstitial nephritis caused by immune checkpoint inhibitors. This combination therapy may offer a therapeutic advantage for patients with stage III or IV ICI-AIN.

Background:

  • Acute interstitial nephritis (AIN) is a frequent adverse effect of immune checkpoint inhibitors (ICIs).
  • Elevated tumor necrosis factor-alpha (TNF-α) expression is noted in ICI-induced AIN (ICI-AIN).

Purpose of the Study:

  • To compare the efficacy of glucocorticoids plus infliximab versus glucocorticoids alone in treating stage III or IV ICI-AIN.
  • To investigate gene expression profiles in ICI-AIN and their association with TNF-α levels.

Main Methods:

  • Retrospective analysis of 55 patients with stage III or IV ICI-AIN.
  • Comparison of treatment groups: steroids only (27 patients) vs. steroids plus infliximab (28 patients).
  • NanoString analysis of kidney samples to evaluate inflammatory gene signatures.

Main Results:

  • Infliximab combined with steroids significantly improved time to renal response (TTR) within 30 days compared to steroids alone.
  • Duration of renal response (DORR) was longer in the infliximab group in univariate analysis.
  • NanoString analysis revealed higher immune expression in ICI-AIN, with elevated TNF-α correlating with renal response.

Conclusions:

  • Infliximab in combination with glucocorticoids may provide a therapeutic benefit for patients with stage III or IV ICI-AIN.
  • TNF-α expression is linked to renal response in ICI-induced AIN.
