Summary

For metastatic castration-resistant prostate cancer (mCRPC), docetaxel rechallenge offers improved overall survival compared to cabazitaxel in patients who did not progress on initial docetaxel. This suggests docetaxel rechallenge is a viable treatment option.

Background:

  • Docetaxel is a standard treatment for metastatic castration-resistant prostate cancer (mCRPC).
  • Cabazitaxel, a related taxane, is approved for mCRPC patients previously treated with docetaxel.
  • The comparative effectiveness of docetaxel rechallenge versus cabazitaxel after prior docetaxel for mCRPC is not well-established.

Purpose of the Study:

  • To compare clinical outcomes of docetaxel rechallenge versus cabazitaxel.
  • To evaluate patients with mCRPC who did not experience disease progression during prior docetaxel treatment.

Main Methods:

  • Retrospective cohort study in the Veterans Affairs health care system.
  • Used inverse probability of treatment weighting to control for confounders.
  • Included patients with chemonaive mCRPC treated with initial docetaxel without progression.

Main Results:

  • Docetaxel rechallenge (262 patients) showed significantly longer overall survival (median 12.3 months) compared to cabazitaxel (407 patients, median 9.6 months).
  • Hazard ratio for overall survival was 0.81 (95% CI, 0.55-0.99; P=.04) favoring docetaxel rechallenge.
  • Secondary outcomes, including prostate-specific antigen response and time to next treatment or death, were also favorable for docetaxel rechallenge.

Conclusions:

  • Docetaxel rechallenge is associated with improved overall survival compared to cabazitaxel.
  • Findings support docetaxel rechallenge as a treatment option for mCRPC patients who did not progress on prior docetaxel.
  • Further research may explore optimal sequencing of taxane-based therapies in mCRPC.
