[A Case of Intraperitoneal Gastrointestinal Stromal Tumor with Pedicle Torsion Requiring Emergency Surgery]

The patient was a 66-year-old woman who presented with sudden-onset lower abdominal pain the day before, followed by intermittent pain, which led to her emergency transport to our hospital. Contrast-enhanced computed tomography revealed a suspected pelvic mass, along with signs consistent with hemorrhage and abscess formation. Based on the possibility of ovarian tumor torsion and her history of abdominal surgery, emergency surgery was performed for both diagnostic and therapeutic purposes due to suspected strangulated intestinal obstruction. Laparoscopic exploration revealed a tumor originating from the pelvic peritoneum that had undergone torsion and rupture. The resected tumor measured 8 cm in diameter and was diagnosed as a gastrointestinal stromal tumo(r GIST). Due to the tumor rupture, the patient was classified as high-risk according to the modified Fletcher classification. Postoperatively, she has continued oral imatinib therapy and remains recurrence-free. Extra-gastrointestinal stromal tumors(EGISTs), which originate outside the gastrointestinal tract, are challenging to diagnose preoperatively. They may present with torsion or rupture, requiring emergency surgical intervention. Because EGISTs are rare entities, we report this case with a review of the relevant literature.
