Gastric heterotopic pancreas (HP) often mimics submucosal tumors like GISTs on imaging. Multidisciplinary integration of endoscopy, EUS-FNA, and histopathology is essential to avoid misdiagnosis. For symptomatic HP, minimally invasive resection is curative; asymptomatic cases require long-term surveillance due to malignant potential.
