Insights

Women who experienced sexual abuse before age 10 reported higher acute postoperative pain and increased prescription opioid use after cesarean delivery. This pain persisted for up to six weeks postpartum.

Area of Science:

  • Obstetrics and Gynecology
  • Pain Medicine
  • Trauma and Abuse Research

Background:

  • Sexual abuse before age 10 affects 2.7% of US women.
  • While chronic pain is linked to sexual abuse, acute postoperative pain in survivors without prior chronic pain is less understood.
  • This study investigates the impact of prepubertal sexual abuse on acute pain following cesarean delivery.

Purpose of the Study:

  • To evaluate if patients reporting prepubertal sexual abuse experience increased acute pain 7 days after hospital discharge from cesarean delivery.
  • To assess the association between prepubertal sexual abuse and postoperative pain, pain catastrophizing, physical function, and opioid use.

Main Methods:

  • Secondary analysis of a multicenter randomized trial involving 4881 participants undergoing cesarean delivery.
  • Prepubertal sexual abuse history was self-reported; exclusion criteria included chronic pain or missing data.
  • Primary outcome: moderate-to-severe worst pain (Brief Pain Inventory [BPI] ≥4) at 7 days post-discharge. Secondary outcomes included pain scores at 2, 6 weeks, and 90 days, Pain Catastrophizing Scale (PCS), Physical Function (PF), and opioid consumption (MME, prescriptions, tablets).

Main Results:

  • 5.3% of participants reported prepubertal sexual abuse and had higher rates of obesity, family substance use disorders, and depression.
  • Individuals reporting prepubertal sexual abuse showed significantly higher moderate-to-severe pain at 7 days post-discharge (75% vs. 57%; aRR 1.3) and at 2 weeks (42% vs. 30%; aRR 1.3) and 6 weeks (14% vs. 10%; aRR 1.5).
  • Prepubertal sexual abuse was associated with higher opioid use before discharge (median MME 22.5 vs. 15), more opioid prescriptions post-discharge (aRR 1.7), and greater total opioid tablet use (9 vs. 4 tabs).

Conclusions:

  • Prepubertal sexual abuse is linked to significantly higher acute postoperative pain through six weeks postpartum following cesarean delivery.
  • Individuals with a history of prepubertal sexual abuse exhibited increased prescription opioid use after cesarean delivery.
  • These findings highlight the need for further research into managing acute pain and opioid use in postpartum individuals with a history of sexual abuse.

An Experimental Paradigm for the Prediction of Post-Operative Pain (PPOP)

Diffuse noxious inhibitory control, temporal summation and wound hyperalgesia testing are demonstrated in the obstetric patient. These tests evaluate inhibitory and excitatory mechanisms of pain processing and are here utilized to evaluate endogenous analgesia at different time-points during pregnancy and the peripartum period to help reveal individual s risk for persistent...

Quantifying Pain Location and Intensity with Multimodal Pain Body Diagrams

Current pain scales used to quantify pain severity, such as visual analog scales, fail to capture the complexity of subjective pain experiences. Pain body diagrams are qualitative but may be more informative. The goal of this method is to extract quantitative metrics from pain body diagrams using novel pressure-hue...
