Ovarian ectopic pregnancy is an exceptionally rare entity, typically diagnosed in the first trimester following rupture. Progression to term is extraordinarily uncommon and associated with significant diagnostic and management challenges. Clinicians should maintain awareness of atypical ectopic presentations and carefully assess adnexal structures intraoperatively to avoid missed diagnoses.
