Terrible APS - a newly defined variant of severe APS

Background:

• Antiphospholipid syndrome (APS) has diverse clinical presentations.
• Some APS patients experience severe, refractory disease not meeting catastrophic APS (cAPS) criteria.

Purpose of the Study:

• To define and characterize a new subcategory of APS, termed Terrible APS (TrAPS).
• To develop a predictive score (TrAPScore) for identifying TrAPS patients.

Main Methods:

• Analysis of 306 primary APS patients, excluding obstetric, non-criteria APS, or cAPS.
• TrAPS definition: >2 breakthrough thrombotic events despite anticoagulation or need for immunomodulation/surgery.
• Multivariate analysis to identify predictors for the TrAPScore.

Main Results:

• 27 (12.7%) of 209 thrombotic primary APS patients met TrAPS criteria.
• TrAPS patients showed higher rates of venous thrombosis, microvascular involvement, heart valve disease, thrombocytopenia, and triple-positive antibodies.
• TrAPS was linked to increased mortality (18.5% vs. 5.1%) and anticoagulant resistance (81.4%).
• TrAPScore predictors: severe thrombocytopenia, heart valve disease, microvascular manifestations, triple-positive serology.
• TrAPScore >6 showed high PPV (78%-82.5%) for TrAPS; score <4 had high NPV (96.9%).

Conclusions:

• Terrible APS (TrAPS) is a distinct, refractory subcategory of APS.
• The TrAPScore effectively identifies patients with severe, refractory disease based on key clinical and serological factors.
