Terrible APS - a newly defined variant of severe APS

Researchers identified a new subcategory of Antiphospholipid Syndrome (APS) called Terrible APS (TrAPS), characterized by recurrent thrombosis despite anticoagulation. A new scoring system, TrAPScore, helps identify patients with this severe, refractory disease.

- TrAPS was linked to increased mortality (18.5% vs. 5.1%) and anticoagulant resistance (81.4%).
- TrAPScore predictors: severe thrombocytopenia, heart valve disease, microvascular manifestations, triple-positive serology.
- TrAPScore >6 showed high PPV (78%-82.5%) for TrAPS; score <4 had high NPV (96.9%).
Conclusions:
- Terrible APS (TrAPS) is a distinct, refractory subcategory of APS.
- The TrAPScore effectively identifies patients with severe, refractory disease based on key clinical and serological factors.
