Umbilical Endometriosis Presenting as Cyclical Umbilical Bleeding: A Case Report

Endometriosis is a benign, estrogen-dependent condition affecting 6%-10% of women of reproductive age, typically involving pelvic organs but occasionally presenting in extrapelvic locations such as the umbilicus. We report a case of umbilical endometriosis in a nulliparous woman who developed cyclic painful bleeding and umbilical swelling 8 months after a diagnostic laparoscopy for infertility. Surgical excision with primary closure was performed, and histopathology, including positive CD10 immunostaining, confirmed the diagnosis. Umbilical endometriosis (Villar's node) is rare, accounting for approximately 0.5%-1% of endometriosis cases. Diagnosis is primarily clinical-based on cyclical umbilical pain and bleeding-and should be considered in women presenting with an umbilical mass. Complete surgical excision remains the treatment of choice, providing excellent outcomes with a low risk of recurrence or malignant transformation.
