Abstract:
Ischemic stroke results from the occlusion of an intracranial artery and requires rapid arterial recanalization to restore cerebral reperfusion. Arterial recanalization treatments, intravenous thrombolysis and mechanical thrombectomy, have improved prognosis, but their accessibility is restricted and their effectiveness in achieving cerebral reperfusion and ensuring good outcome remains limited. Recent studies have identified thromboinflammation as a key factor in resistance to these treatments, particularly by limiting the effectiveness of intravenous thrombolysis in achieving arterial recanalization, but also by preventing arterial recanalization from resulting in cerebral reperfusion. In this review, we present how thromboinflammation contributes to resistance to arterial recanalization treatments and discuss the opportunities offered by treatments targeting its mechanisms to optimize patient management and prognosis.
