Background:

  • Localized prostate cancer (PCa) management options include radical treatments and minimally invasive focal therapies.
  • Focal cryotherapy is an emerging option for localized PCa, but its outcomes relative to baseline disease severity require further investigation.

Purpose of the Study:

  • To evaluate the oncological outcomes of focal cryotherapy for localized prostate cancer (PCa).
  • To assess the impact of baseline Gleason Grade Group (GG) on recurrence-free survival after focal cryotherapy.

Main Methods:

  • Retrospective analysis of 111 patients with localized PCa treated with focal cryotherapy (2014-2024).
  • Exclusion criteria included prior treatments or <12 months follow-up; all patients had MRI and transperineal biopsy.
  • Recurrence-free and radical treatment-free survival were analyzed, comparing outcomes between baseline GG 1 and GG >1 groups.

Main Results:

  • Median follow-up was 35 months; 78% of patients had confirmatory biopsies.
  • In-field recurrence occurred in 10% and out-field recurrence in 23% of patients.
  • No significant differences in recurrence rates or three-year recurrence-free survival (63%) or radical treatment-free survival (85%) were observed between baseline GG groups.

Conclusions:

  • Focal cryotherapy demonstrates favorable short-term oncological outcomes for localized prostate cancer.
  • Baseline Gleason score does not appear to significantly influence recurrence-free survival following focal cryotherapy.
