Purpose of the Study:

  • A population-based cohort study of 37,520 patients diagnosed with BC in Ontario, Canada, between January 2018 and December 2021.
  • Patients were categorized into pre-pandemic, immediate pandemic, and peri-pandemic groups.
  • Time-to-event analysis was used to evaluate time to first BC surgery, with subgroup analyses for neoadjuvant chemotherapy (NAC) and neoadjuvant endocrine therapy (NET).

  • The probability of undergoing BC surgery monthly was significantly lower during the immediate and peri-pandemic periods compared to the pre-pandemic period.

  • For patients receiving surgery as the first treatment, the median wait time was shortest during the immediate pandemic (1.0 month) compared to pre- and peri-pandemic periods (1.2 months).

Conclusions:

  • The COVID-19 pandemic resulted in longer diagnosis-to-surgery times for breast cancer patients.
  • Increased use of neoadjuvant therapies was observed during the pandemic.
  • Despite overall longer wait times, surgical wait times post-neoadjuvant therapy were shorter during the immediate pandemic, indicating a triage of care.
