Summary

This pilot study on acute ischemic stroke patients found that individualized blood pressure management during endovascular therapy was not feasible due to difficulties in maintaining target blood pressure. Further trials with the current protocol are not recommended.

Purpose of the Study:

  • To assess the feasibility of two blood pressure management strategies in patients undergoing endovascular therapy for acute ischemic stroke.

Main Methods:

  • A randomized controlled pilot trial involving 60 adult patients with acute ischemic stroke and large-vessel occlusion.
  • Patients were randomized to standard (mean arterial blood pressure [MABP] 70-90 mm Hg) or individualized (MABP ±10% of baseline) targets.
  • Primary outcome was the modified Rankin Scale score at 90 days; secondary outcomes included feasibility metrics and reperfusion rates.

Main Results:

  • The individualized blood pressure group showed significantly higher mean arterial blood pressure and systolic blood pressure compared to the standard group.
  • Full reperfusion rates were similar between groups (90% vs. 93%).
  • Feasibility criteria were not met, as neither group consistently stayed within their target MABP range (54.3% vs. 61.4% of time outside targets).

Conclusions:

  • Difficulties in achieving target blood pressure made the individualized strategy infeasible in this pilot study.
  • The findings suggest that the current protocol is not suitable for a larger trial.
  • Further research is needed to refine blood pressure management strategies for endovascular stroke therapy.
