PET-Based Outcome Prediction in Patients with Prostate Cancer Scheduled for [225Ac]Ac-PSMA Radiopharmaceutical

Pre-treatment imaging with [18F]F-PSMA-1007 PET/CT can predict outcomes for patients receiving actinium-225-labeled prostate-specific membrane antigen (PSMA) radiopharmaceutical therapy (RPT). Higher average SUVmean values predict better progression-free survival, while PSMA-total lesion quotient predicts overall survival.

- Pretherapeutic [18F]F-PSMA-1007 PET/CT SUVmean is a strong predictor of early treatment response and PFS in patients receiving [225Ac]Ac-PSMA-I&T RPT.
- PSMA-TLQ is associated with OS, suggesting its utility in predicting long-term outcomes.
- PSMA PET-based quantification can optimize patient selection for targeted alpha-RPT.
