Summary

Serum soluble TREM2 (sTREM2) levels do not reliably predict disability progression in primary progressive multiple sclerosis (PPMS). This study found limited value for sTREM2 as a biomarker in PPMS patients.

Background:

  • Triggering receptor expressed on myeloid cells 2 (TREM2) is a microglia receptor involved in tissue repair.
  • Soluble TREM2 (sTREM2) in blood and CSF indicates microglial activation.
  • Microglia play a key role in multiple sclerosis (MS) pathogenesis.

Purpose of the Study:

  • To investigate if serum sTREM2 levels can predict disability progression in primary progressive MS (PPMS).

Main Methods:

  • Serum sTREM2 levels were measured using a single-molecule array assay in 137 PPMS patients.
  • Linear models assessed the association between sTREM2 and Expanded Disability Status Scale (EDSS) change over time.

Main Results:

  • An association between sTREM2 and disability progression was observed at 2 years, but only in non-inflammatory PPMS patients.
  • No consistent relationship was found between sTREM2 levels and disability progression over longer follow-up periods.

Conclusions:

  • Serum sTREM2 shows limited utility as a biomarker for predicting disability progression in PPMS.
  • Further research may be needed to explore potential roles of sTREM2 in specific MS subgroups.
