Effect of Nurse-Led Telecoaching on Patients With Heart Failure: A Systematic Review and Meta-Analysis

Purpose:
The current systematic review and meta-analysis aimed to investigate the effect of nurse-led telecoaching on anxiety, depression, heart failure (HF) knowledge, self-care, and quality of life of patients with HF.
Method:
We systematically searched PubMed, Cochrane Library, Scopus, and Web of Science from database inception to April 20, 2025, to identify randomized controlled trials comparing the effect of nurse-led telecoaching on anxiety, depression, HF knowledge, self-care, and quality of life of patients with HF. A random-effects model was adopted to pool data, and the revised RoB2 tool was used to assess risk of bias.
Results:
Nineteen studies with 2,917 participants were included. Compared to usual care, nurse-led telecoaching significantly improved HF knowledge of patients with HF (standard mean difference [SMD] = 2.09, 95% confidence interval [CI] [0.26, 3.91], I2 = 99.22%), but its effects on depression (SMD = -0.26, 95% CI [-0.54, 0.01], I2 = 72.12%), anxiety (SMD = -0.06, 95% CI [-0.22, 0.1], I2 = 0), self-care (SMD = 0.12, 95% CI [-0.61, 0.85], I2 = 98.17%), and quality of life (SMD = -0.33, 95% CI [-0.84, 0.19], I2 = 96.77%) did not reach statistical significance.
Conclusion:
Findings indicate that compared to usual care, nurse-led telecoaching only improved patients' HF knowledge, but not anxiety, depression, self-care, or quality of life.
