Modified Colonna Capsular Arthroplasty Procedure to Treat Neglected Developmental Dysplasia of the Hip

Background:
The management of neglected developmental dysplasia of the hip (DDH) in adolescents and young adults presents a significant challenge. This study aimed to assess the clinical and radiological outcomes of the modified Colonna capsular arthroplasty technique in patients with neglected DDH.
MatеRials And Methods:
This retrospective cohort study included four patients (aged 13-24 years) with unilateral neglected DDH who underwent the modified Colonna capsular arthroplasty procеdurе between 2012 and 2021. Preoperative and postoperative radiographic parameters were assessed, such as lateral center-edge angle (LCEA), acetabular index (AI), joint space width, and medial head border-ilioischial line distance. Clinical outcomes were evaluated using the Harris Hip Score (HHS), Postel-Merle d'Aubigné (PMA) score, and range of motion (ROM).
Results:
The mean follow-up duration was 40.75 months (12-90 months). Postoperatively, the mean LCEA was 36° (range: 29-45°), and the mean AI was -0.5° (range: -6 to 4°). The joint space width was 2.5 mm (range: 1-4 mm), and the medial head border-ilioischial line distance was 4.5 mm (range: 3-8 mm). The mean postoperative PMA score was 17 (range: 16-18), and the mean HHS was 96.5 (range: 93-100). All patients achieved normal hip ROM, with no reported cases of avascular necrosis or femoral neck fractures. One patient developed class II heterotopic ossification; however, no further intervention was required.
Conclusions:
Modified Colonna capsular arthroplasty seems to be a reliable and efficient treatment for neglected DDH in young patients, providing satisfactory joint preservation, improved function, and minimal complications.
