Background:

  • Childhood cancer survivors have an elevated risk of cardiovascular disease (CVD).

Purpose of the Study:

  • To evaluate if a survivorship care plan (SCP)-based counseling intervention improves CVD risk factor control in high-risk adult survivors of childhood cancer.
  • To assess the impact of personalized risk information and management planning on CVD risk factors.

Main Methods:

  • A randomized clinical trial (Communicating Health Information and Improving Coordination With Primary Care [CHIIP] Study) involving 347 adult survivors of childhood cancer with undertreated CVD risk factors.
  • Participants were randomized to either an SCP-based counseling intervention or enhanced care (control).
  • Intervention included remote counseling sessions and a personalized management plan; controls received basic feedback and encouragement to consult primary care clinicians (PCCs).

Main Results:

  • The SCP-based counseling intervention did not significantly reduce undertreatment of CVD risk factors compared to the enhanced care control group (odds ratio, 1.31; 95% CI, 0.84-2.05).
  • However, greater participant engagement in the intervention was associated with reduced undertreatment at 1 year (odds ratio, 0.31; 95% CI, 0.18-0.72).
  • The intervention group showed improved PCC documentation of CVD risk (14.8% vs 0.9%; P=.002).

Conclusions:

  • The addition of survivorship-based self-management counseling did not significantly reduce undertreatment of CVD risk factors beyond standard risk assessments and communication with primary care clinicians.
  • Further research is needed to identify effective strategies for mitigating CVD risk in high-risk childhood cancer survivors.
  • Enhanced engagement with the intervention showed promise in reducing undertreatment, suggesting potential for tailored approaches.
