Purpose of the Study:

  • To develop and validate a simplified, user-friendly prediction tool for metastatic recurrence risk.
  • To aid in the selection of adjuvant therapies and clinical trial design for non-metastatic RCC with venous tumor thrombus.

Main Methods:

  • A cohort of 532 patients undergoing nephrectomy and venous thrombectomy for M0 RCC was analyzed.
  • A nomogram was developed using LASSO regression and validated internally and externally.
  • Decision curve analysis was performed to assess the utility of adjuvant Pembrolizumab.

Main Results:

  • The nomogram incorporated thrombus level, tumor necrosis, sarcomatoid features, and pN stage.
  • Five-year AUCs were 0.74 (internal) and 0.71-0.68 (external validation).
