Fully digital CAD/CAM milling of lithium disilicate crowns offers superior marginal accuracy compared to traditional heat-press methods. Using computer-aided design (CAD) milled wax patterns also significantly improves crown fit over manual waxing techniques.
Area of Science:
- Dental Materials Science
- Prosthodontics
- Digital Dentistry
Background:
- Marginal precision of all-ceramic crowns is critical for clinical longevity.
- Discrepancies can lead to cement dissolution, microleakage, secondary caries, and periodontal issues.
- Lithium disilicate is a popular material for all-ceramic restorations.
Purpose of the Study:
- To evaluate and compare the vertical marginal precision of lithium disilicate crowns fabricated using three different methods:
- 1. Conventional heat-press with manual waxing.
- 2. Heat-press with CAD-milled wax patterns.
- 3. Fully digital CAD/CAM milling.
Main Methods:
- Thirty lithium disilicate crowns were fabricated on a master die.
- Groups were: conventional heat-press (n=10), heat-press with CAD-milled wax (n=10), and fully digital CAD/CAM (n=10).
- Vertical marginal gaps were measured using a stereomicroscope at 20x magnification.
Main Results:
- Significant differences in marginal gap were found between the groups (p=0.0001).
- The fully digital CAD/CAM group exhibited the smallest mean marginal gap.
- Both digital methods showed improved marginal accuracy compared to conventional heat-pressing.
Conclusions:
- Fully digital CAD/CAM milling of lithium disilicate provides superior vertical marginal accuracy.
- CAD-milled wax patterns enhance fit compared to conventional manual waxing.
- Digital workflows offer improved precision for lithium disilicate crowns.
