Summary

Chronic hand eczema (CHE) is difficult to treat. This study found that a detailed clinical evaluation and a new case report form (CRF) help classify patients, enabling personalized treatment strategies for better outcomes.

Background:

  • Chronic hand eczema (CHE) presents complex multifactorial causes and varied clinical signs.
  • CHE often shows resistance to standard therapeutic interventions.

Purpose of the Study:

  • To conduct a clinical and etiological investigation of patients with chronic hand eczema.
  • To evaluate the utility of a specifically developed case report form (CRF) for CHE assessment.

Main Methods:

  • A cross-sectional study involving 207 adult patients across 10 Italian dermatology clinics.
  • Data collection included demographics, clinical presentation, disease severity, risk factors, patch testing, treatment response, and disease burden using a dedicated CRF.
  • Analysis of recorded data to identify etiological and clinical subtypes of CHE.

Main Results:

  • The study enrolled 207 patients (68.6% female, mean age 41.1 years).
  • 58.9% of CHE cases were moderate-severe and 81.4% were refractory to potent topical corticosteroids.
  • Etiological subtypes included irritant contact dermatitis (52.7%), allergic contact dermatitis (24.2%), and atopic dermatitis (16.9%). Clinical subtypes were identified in only 29.0% of patients.
  • The developed CRF was found to be user-friendly and effective.

Conclusions:

  • Comprehensive clinical workup allows for CHE classification in approximately 80% of cases.
  • Accurate etiological and clinical classification facilitates the development of tailored treatment strategies for chronic hand eczema.
