Parainguinal hernias (PHs) are often misdiagnosed but are more common than Spigelian hernias (SHs). Open mesh repair for PHs shows low complications and excellent long-term outcomes.

Background:

  • Parainguinal hernias (PHs) are an under-recognized lateral abdominal wall hernia subtype.
  • PHs are frequently misclassified as Spigelian hernias (SHs) or inguinal hernias due to overlapping clinical features.
  • Accurate diagnosis and tailored management are crucial for effective treatment.

Purpose of the Study:

  • To define the clinical characteristics of PHs.
  • To compare the diagnostic accuracy of PHs versus SHs.
  • To evaluate operative management and long-term outcomes for PHs and SHs.

Main Methods:

  • Retrospective cohort study of adult patients undergoing mesh repair for PHs or SHs (2002-2025).
  • Analysis of clinical, radiological, and operative findings, complications, and long-term outcomes.
  • Pain outcomes assessed using Cunningham's criteria.

Main Results:

  • PHs were more common than SHs (40 vs. 5 cases) and frequently misdiagnosed preoperatively (51%).
  • Clinical diagnosis of PHs had moderate sensitivity (56%) but high positive predictive value (96%); imaging showed poor sensitivity.
  • Open or laparoscopy-assisted mesh repair for PHs yielded excellent long-term outcomes with low recurrence (4%) and complication rates (seroma 4%).

Conclusions:

  • Parainguinal hernias are more prevalent than Spigelian hernias and warrant recognition as a distinct entity.
  • Improved diagnostic strategies are needed to differentiate PHs from other hernia types.
  • Open extraperitoneal mesh repair for PHs is associated with low morbidity and excellent long-term results.
