Levodopa treatment did not impact post-stroke depression (PSD) rates in patients undergoing rehabilitation. This randomized trial found no significant difference in PSD occurrence between levodopa and placebo groups at three months.
Area of Science:
- Neuroscience
- Clinical Neurology
- Psychiatry
Background:
- Post-stroke depression (PSD) is a common complication affecting stroke patient recovery.
- Dopamine plays a role in mood and motivation, and low levels are associated with PSD.
- Investigating treatments for PSD is crucial for improving patient outcomes.
Purpose of the Study:
- To determine if levodopa treatment influences the incidence of post-stroke depression (PSD).
- To assess the efficacy of levodopa in managing emotional well-being during stroke rehabilitation.
Main Methods:
- A multicenter, randomized, double-blind, placebo-controlled trial (ESTREL-Depression) was conducted.
- Participants received either levodopa/carbidopa or placebo for 39 days during in-hospital rehabilitation.
- PSD was assessed at 3 months using the Patient-Reported Outcomes Measurement Information System (PROMIS) short-form depression-4a.
Main Results:
- The study included 407 participants (209 levodopa, 198 placebo).
- At 3 months, the frequency of PSD was 26% in the levodopa group and 28% in the placebo group.
- Levodopa treatment showed no significant effect on the odds of developing PSD (OR = 0.93).
Conclusions:
- Levodopa treatment did not demonstrate a significant impact on the occurrence of post-stroke depression.
- Further research may be needed to explore alternative therapeutic strategies for PSD.
