Background:

  • Screening mammography false-negative rates (FNRs) are influenced by interval cancers (ICs), which can be detected by supplemental screening (SS).
  • Understanding the impact of SS on FNRs and comparing characteristics of symptomatic ICs versus asymptomatic false-negative (FN) cancers is crucial for optimizing breast cancer screening protocols.

Purpose of the Study:

  • To evaluate the effect of a robust SS program on FNRs for patients at higher-than-average risk.
  • To compare patient and tumor characteristics between symptomatic ICs and asymptomatic FN cancers detected via SS.

Main Methods:

  • Retrospective review of screening mammogram audit metrics from an academic institution (July 2018-June 2023).
  • Identification of false-negative (FN) cancers as those diagnosed within 12 months of a negative screening mammogram.
  • Analysis of patient risk, detection method, SS utilization, and tumor characteristics using Cochran-Mantel-Haenszel and univariable tests.

Main Results:

  • Out of 106,750 mammograms, 624 cancers and 119 FN cancers were identified (FNR: 1.11/1000).
  • Symptomatic ICs (45%) were more frequent in patients with a personal history of breast cancer and higher risk (74%).
  • Asymptomatic FNs were predominantly detected by MRI (60%), and symptomatic ICs were larger at diagnosis (2.3 cm vs 1.5 cm).

Conclusions:

  • A robust SS program can lower the symptomatic FNR below national benchmarks.
  • Higher-risk patients with symptomatic ICs frequently underutilized SS MRI, indicating a need for improved access and awareness.
