This case report highlights a rare instance of pulmonary Kaposi sarcoma (KS) in an HIV/AIDS patient presenting with respiratory failure. Judicious corticosteroid use, alongside paclitaxel, improved outcomes in this pulmonary KS case.

- Kaposi sarcoma (KS) is an angioproliferative disorder linked to human herpesvirus 8 (HHV-8).
- While KS commonly presents with cutaneous and extracutaneous manifestations (oral, GI, lymph nodes), visceral organ involvement, particularly pulmonary, is exceptionally rare.
- Pulmonary KS can manifest severely, leading to conditions like acute hypoxemic respiratory failure (AHRF).
