Quantifying the Pressure of Labial and Buccinator Regions Pre- and Post- Facial Nerve Reanimation

Background:
Flaccid facial nerve paralysis (FFNP) impairs oral function and quality of life. Tensor fascia lata slings (TFL) are used to improve oral competence. This study used the Iowa Oral Performance Instrument (IOPI) to measure oral competence in patients with FFNP undergoing facial reanimation.
Method:
Between 2022 and 2024, patients with FFNP undergoing TFL sling reconstruction had their intra-labial and buccinator pressures measured using the IOPI. Patient-reported outcomes were collected with the Oral Competence Questionnaire (OCQ) and Speech Handicap Index (SHI). Pre- and post-surgical data were analyzed with Spearman's correlation and Wilcoxon testing.
Results:
Significant correlations were found between IOPI values and OCQ (r ≈ -0.54, p < 0.001) and SHI (r ≈ -0.35, p < 0.05). Baseline IOPI values were lower on the flaccid side. Post-surgery, OCQ and SHI improved significantly.
Conclusions:
IOPI measures correlate with patient-reported outcomes and provide an objective adjunct for assessing oral function in FFNP reconstruction.
