Summary

The L-OPN technique, combining contrast-enhanced excimer laser coronary atherectomy (ELCA) with OPN balloon angioplasty, offers a viable solution for complex multilayer in-stent restenosis (ISR) when other methods fail. Intravascular imaging guidance is crucial for successful outcomes in these challenging cases.
