Summary

Minimally invasive surgery (MIS) may increase recurrence risk for borderline ovarian tumours (BOTs), especially in fertility-sparing procedures. Further research is needed to confirm these findings for surgical counseling.

Background:

  • Borderline ovarian tumours (BOTs) primarily affect women of reproductive age.
  • Concerns exist regarding the oncological safety of minimally invasive surgery (MIS) for BOTs, particularly in fertility-sparing cases.

Purpose of the Study:

  • To evaluate the association between MIS and recurrence risk in BOT patients.
  • To stratify analysis by fertility-sparing status to assess differential effects.

Main Methods:

  • Single-centre cohort study of 91 BOT patients (2014-2023).
  • Comparison of progression-free survival between MIS and open surgical approaches.

Main Results:

  • MIS was associated with a higher observed recurrence rate compared to open surgery (20.0% vs. 4.5%).
  • An absolute risk difference of 15.5% and an unadjusted hazard ratio of 5.29 were observed (p=0.022).

Conclusions:
