Background:

  • Minimally invasive hysterectomy with sentinel lymph node (SLN) mapping is standard for early-stage endometrial cancer.
  • Comparative real-world data on robot-assisted hysterectomy (RAH) versus traditional laparoscopy hysterectomy (TLH) are limited.

Purpose of the Study:

  • To compare the real-world effectiveness of RAH versus TLH in early-stage endometrial cancer.
  • To evaluate SLN detection rates and conversion rates between RAH and TLH.

Main Methods:

  • Retrospective review of 140 patients with FIGO stage I endometrial cancer undergoing RAH or TLH.
  • Analysis of patient demographics, SLN detection rates, conversion rates, operative time, and pathological characteristics.

Main Results:

  • Robot-assisted hysterectomy (RAH) demonstrated a significantly higher SLN detection rate (98%) compared to traditional laparoscopy hysterectomy (TLH) (90.2%).
  • Conversion to laparotomy was significantly lower in the RAH group (0%) versus the TLH group (11.5%).
  • No significant differences were observed in operative time or key pathological parameters.

Conclusions:

  • In a standardized ICG-guided SLN mapping setting, RAH achieved superior SLN detection compared to TLH.
  • RAH resulted in markedly lower conversion rates than TLH for early-stage endometrial cancer.
  • RAH is a viable alternative to TLH with improved outcomes in SLN mapping.
