Background:

  • End-stage kidney disease (ESKD) is increasing in Pakistan, a resource-limited nation.

Purpose of the Study:

  • To report the outcomes of the largest ABO-incompatible kidney transplantation program in Pakistan.
  • To assess the feasibility and effectiveness of ABO-incompatible kidney transplantation in a resource-limited setting.

Main Methods:

  • Implementation of a large-scale ABO-incompatible kidney transplant program starting February 2023.
  • Pretransplant desensitization protocol including rituximab, tacrolimus, mycophenolic acid, steroids, and apheresis.
  • Monitoring of graft function, patient survival, rejection episodes, and complications over a median follow-up of 12 months.

Main Results:

  • 21 ABO-incompatible kidney transplants were performed.
  • Excellent short-term graft survival (no graft loss) with a median serum creatinine of 1.1 mg/dL.
  • Acute rejection occurred in 3 patients (14.3%) and was successfully treated; 2 infectious complications led to death.

Conclusions:

  • ABO-incompatible living donor kidney transplantation is feasible and effective in Pakistan, offering excellent short-term graft outcomes.
  • Infectious and cardiac complications represent significant challenges requiring careful management.
  • This approach expands kidney transplantation options in resource-limited countries.
