Background:
Biologic therapies are recommended for severe, uncontrolled chronic rhinosinusitis with nasal polyps (CRSwNP), but evidence in moderate disease remains limited.
Objective:
To evaluate the effectiveness of omalizumab in patients with moderate CRSwNP and mild-to-moderate IgE-mediated asthma without prior sinus surgery.
Methods:
In this randomized, controlled, open-label study, adult patients were recruited through integrated otolaryngology and allergy clinics; 59 patients were randomized (3:2) to omalizumab plus standard therapy (n = 36) or standard therapy alone (n = 23) and followed for 12 months. The primary end point was change in Nasal Polyp Score. Secondary end points included nasal obstruction, asthma control, exacerbations, systemic corticosteroid use, and biomarkers of type 2 inflammation.
Results:
Omalizumab significantly reduced Nasal Polyp Score, improved nasal obstruction and asthma control, reduced asthma exacerbations and systemic corticosteroid exposure, and improved biomarkers of type 2 inflammation compared with controls.
Conclusion:
Omalizumab is effective in patients with moderate CRSwNP and mild-to-moderate asthma and may have a role earlier in the treatment algorithm for selected patients, pending confirmation in larger, multicenter studies.
Insights
Omalizumab effectively treats moderate chronic rhinosinusitis with nasal polyps (CRSwNP) and mild-to-moderate asthma. This biologic therapy improves symptoms and reduces exacerbations, suggesting earlier use in treatment algorithms.
Area of Science:
- Otolaryngology
- Allergy and Immunology
- Pulmonology
Background:
- Limited evidence exists for biologic therapies in moderate chronic rhinosinusitis with nasal polyps (CRSwNP).
- Biologics are typically recommended for severe, uncontrolled CRSwNP.
- Investigating omalizumab's efficacy in moderate CRSwNP is crucial.
Purpose of the Study:
- To evaluate the effectiveness of omalizumab in patients with moderate CRSwNP and mild-to-moderate IgE-mediated asthma.
- To assess omalizumab's impact on nasal polyp score, obstruction, and asthma control.
- To explore effects on exacerbations, corticosteroid use, and type 2 inflammation biomarkers.
Main Methods:
- A randomized, controlled, open-label study involving 59 adult patients.
- Patients received either omalizumab plus standard therapy or standard therapy alone for 12 months.
- Primary endpoint: change in Nasal Polyp Score (NPS); secondary endpoints included asthma control and biomarkers.
Main Results:
- Omalizumab significantly reduced NPS compared to controls.
- Significant improvements were observed in nasal obstruction, asthma control, and type 2 inflammation biomarkers.
- Omalizumab use led to reduced asthma exacerbations and systemic corticosteroid exposure.
Conclusions:
- Omalizumab demonstrates efficacy in moderate CRSwNP patients with co-existing mild-to-moderate asthma.
- The findings suggest a potential role for omalizumab earlier in the treatment algorithm for selected patients.
- Larger, multicenter studies are needed to confirm these results.

Antiasthma Drugs: Mast Cell Stabilizers and Anti-IgE Drugs
Mast cell stabilizers, such as cromolyn (also known as sodium cromoglycate) and nedocromil (Tilade), are effective drugs in asthma management. These stabilizers hinder histamine release by skillfully obstructing the activation of mast cells and other cellular entities. Notably, they navigate this task without...

Asthma: Pathogenesis and Management
Asthma is classified as allergic and non-allergic. Allergens such as dust mites, pollen, and pet dander trigger allergic asthma, while factors like cold air, intense emotions, or exercise can induce non-allergic asthma.
