Abstract:
Pelvic arteriovenous malformation compressing the prostate is a rare vascular anomaly mimicking benign prostatic hyperplasia. We reported a 59-year-old male with progressive lower urinary tract symptoms. Imaging revealed a 27 × 35 mm vascular mass compressing the prostate, consistent with a pelvic arteriovenous malformation supplied by the right prostatic artery and branches of the right internal iliac artery. Selective transarterial embolization using N-butyl-cyanoacrylate and lipiodol achieved complete occlusion. The patient was discharged the following day. At 1 month, the symptoms had resolved, and computed tomography demonstrated thrombosis and reduction of the venous sac. At 6 months, the patient remained asymptomatic, and ultrasound confirmed further regression without recurrence. This case highlights pelvic arteriovenous malformation as an uncommon cause of lower urinary tract symptoms and supports embolization as a safe, effective treatment.
