Purpose of the Study:

  • To investigate if the antiplatelet effects of prasugrel and clopidogrel differ based on patient characteristics.
  • To analyze these differences within the context of the ACUTE-PRAS study.

Main Methods:

  • Post hoc analysis of the ACUTE-PRAS randomized controlled trial.
  • 176 patients with acute atherothrombotic stroke or high-risk TIA were included.
  • High platelet reactivity (HPR) and platelet reaction units (PRU) were assessed on Day 5 in various subgroups.

  • Prasugrel showed numerically lower HPR rates than clopidogrel in specific subgroups.
  • Favorable trends for prasugrel were observed in patients with ABCD-GENE score ≥ 10, CKD, age >75, BMI <25, dyslipidemia, DM, treatment initiation ≤ 24h, and certain NIHSS scores.
  • Prasugrel also reduced PRU more than clopidogrel across most subgroups, with exceptions noted.

Conclusions:

  • Prasugrel offers favorable early platelet inhibition.
  • The drug's efficacy is particularly pronounced in patients with advanced age, CKD, low BMI, metabolic comorbidities, or very early treatment initiation.
  • Findings suggest potential for personalized antiplatelet strategies in stroke patients.
