Summary

Tooth loss (edentulousness) in older adults may increase frailty risk, particularly through loneliness and inflammation (C-reactive protein). Preserving teeth and addressing loneliness could help prevent frailty in seniors.

Background:

Purpose of the Study:

  • To investigate the longitudinal association between edentulousness (complete tooth loss) and frailty in older English adults.
  • To examine potential mediating pathways, including loneliness, C-reactive protein (CRP), and fruit and vegetable consumption.

Main Methods:

  • Utilized data from 6,800 adults aged ≥50 years from the English Longitudinal Study of Ageing (ELSA).
  • Assessed edentulousness and frailty (using the Frailty Index) at waves 3, 5, and 7.
  • Employed a cross-lagged longitudinal structural equation model with Bayesian estimation and probit regression.

Main Results:

  • Edentulousness at wave 3 predicted increased frailty at wave 5, but this association was not significant between waves 5 and 7.
  • Frailty did not significantly predict edentulousness in either time period.
  • Loneliness and CRP showed significant, albeit small, indirect effects on the edentulousness-frailty association; fruit and vegetable intake did not.

Conclusions:

  • Tooth loss may contribute to frailty development in older adults, with loneliness and inflammation playing a role.
  • Public health initiatives focusing on maintaining natural teeth and mitigating loneliness may help prevent frailty.
