Effects of Comprehensive Geriatric Assessment-Guided Interventions on Fall Prevention in High-Risk Older Adults: A

Purpose:
To evaluate the effects of comprehensive geriatric assessment (CGA)-guided fall prevention interventions among older adults at high risk of falling, and identify key elements of these interventions across hospital, community, and residential care home settings.
Method:
A literature search was conducted in six English and Chinese databases up to September 2025, focusing on randomized controlled trials (RCTs). Twenty-seven studies met inclusion criteria.
Results:
Meta-analysis demonstrated that CGA-guided interventions significantly reduced the incidence of fallers, recurrent falls, and hospitalization rates, particularly within community settings, whereas no significant differences were observed in injury or fracture rates. These interventions incorporated comprehensive assessments and targeted strategies addressing physical health, mental health, nutrition, medication management, physical activity or exercise, behavioral approaches, and environmental modifications. Moreover, management of visual and hearing impairments, promotion of social engagement, facilitation of access to community resources, and encouragement of safe physical activity-with an emphasis on the essential role of family support-were highlighted in community-based contexts.
Conclusion:
CGA-guided interventions significantly reduce falls and hospitalizations among high-risk older adults, especially in community settings. Using multidimensional approaches is essential for effective fall prevention. Findings offer valuable guidance for nurses and health care professionals in customizing fall prevention strategies across diverse care settings.
