Summary

Exudative retinal detachment (ERD) is a rare but significant finding in Acute Retinal Necrosis (ARN), indicating severe inflammation. While ERD resolved in all patients, half developed rhegmatogenous retinal detachment.

Background:

  • Acute Retinal Necrosis (ARN) is a severe viral retinitis.
  • Exudative retinal detachment (ERD) is an uncommon complication of ARN.

Purpose of the Study:

  • To analyze the clinical characteristics and outcomes of patients with ARN presenting with ERD.
  • To understand the implications of ERD in ARN patients.

Main Methods:

  • Retrospective observational study of ARN cases with ERD.
  • Data collected from tertiary eye care centers in India (2015-2024).
  • Reviewed clinical records of 322 ARN cases, focusing on 21 eyes with ERD.

Main Results:

  • ERD occurred in 4.34% of ARN cases (21 eyes/14 patients).
  • ARN with ERD showed extensive involvement (90% four quadrants) and high rates of occlusive vasculitis (90%).
  • ERD resolved in all patients within ~11 days, but 50% developed subsequent rhegmatogenous retinal detachment (RRD).

Conclusions:

  • ERD in ARN signifies extensive disease activity and severe inflammation.
  • Eyes with ERD exhibit higher rates of occlusive vasculitis and more severe retinal involvement.
  • ERD is a critical indicator of aggressive ARN, often preceding RRD.
