Summary

Preoperative positron emission tomography-computed tomography (PET-CT) did not reduce the rate of video-assisted thoracoscopic surgery (VATS) lobectomy for benign lung nodules. Younger patients undergoing VATS lobectomy face a higher risk of operating on unsuspected benign disease.

Background:

  • Accurate preoperative characterization of solitary pulmonary nodules is crucial to avoid unnecessary surgeries.
  • Distinguishing early lung neoplasms from benign lesions prevents unwarranted anatomical resections.
  • Video-assisted thoracoscopic surgery (VATS) lobectomy is a common procedure for lung nodules.

Purpose of the Study:

  • To evaluate the impact of preoperative positron emission tomography-computed tomography (PET-CT) on the incidence of VATS lobectomy for unsuspected benign lung disease.
  • To identify risk factors associated with VATS lobectomy for benign lesions in patients without a preoperative diagnosis.

Main Methods:

  • A retrospective study included 864 patients who underwent VATS lobectomy without a preoperative diagnosis.
  • Propensity score matching created comparable groups: 210 patients with preoperative PET-CT and 210 without.
  • Univariate and multivariate analyses assessed factors influencing benign postoperative diagnosis.

Main Results:

  • No significant difference in benign postoperative diagnoses was observed between the PET-CT and non-PET-CT groups (2.9% vs. 4.8%, P=0.31).
  • Younger age (P=0.02) and absence of chronic obstructive pulmonary disease (COPD) (P=0.046) were associated with a higher risk of benign postoperative diagnosis.
  • Younger age was the sole significant risk factor for VATS lobectomy for benign lesions (OR=1.062, P=0.042).

Conclusions:

  • Preoperative PET-CT did not reduce the rate of VATS lobectomy for unsuspected benign lung tumors.
  • The risk of operating on benign lesions during VATS lobectomy should be considered, particularly in younger patients.
  • Further research may explore improved preoperative diagnostic strategies for solitary pulmonary nodules.
