Hyperthyroidism I: Introduction

30
Hyperthyroidism is a type of thyrotoxicosis characterized by the thyroid gland's overproduction of the thyroid hormones triiodothyronine (T3) and thyroxine (T4). This hormone excess increases the basal metabolic rate and enhances sensitivity to catecholamines.DiagnosisDiagnosis is based on clinical features and biochemical testing. It typically shows suppressed thyroid-stimulating hormone (TSH) levels below 0.4 mIU/L, with elevated free T3 and/or T4. Additional tests, including thyroid...

Summary

Same-day total thyroidectomy was safely implemented in a vulnerable population, achieving high adoption. This approach reduced hospital stays without increasing adverse events, demonstrating its effectiveness.

Area of Science:

  • Endocrinology
  • Surgical Oncology
  • Health Services Research

Background:

  • Same-day total thyroidectomy adoption is limited, especially in safety-net hospitals due to socioeconomic barriers.
  • A prospective study evaluated implementing a protocol for same-day total thyroidectomy at a high-volume, safety-net academic medical center.

Purpose of the Study:

  • To assess the feasibility and safety of a protocolized same-day total thyroidectomy program.
  • To evaluate the impact of same-day discharge on patient outcomes and hospital utilization in a socioeconomically diverse population.

Main Methods:

  • A prospective quality improvement study included patients undergoing total thyroidectomy from January 2023 to December 2024.
  • Eligible patients were offered same-day discharge after a 4-hour observation period with parathyroid hormone-guided calcium management.
  • Outcomes were compared between same-day and overnight discharge cohorts.

Main Results:

  • 36% of patients were discharged same-day (81% of eligible), with adoption increasing from 32% to 88% over the study.
  • Complication rates (nerve injury, hematoma, hypoparathyroidism) were comparable between same-day and overnight cohorts.
  • Same-day discharge significantly reduced median post-operative hospital time (5.1 hours vs. 22.7 hours).

Conclusions:

  • Protocolized same-day total thyroidectomy was safely implemented in a socioeconomically vulnerable population.
  • The program achieved high adoption rates and substantially reduced hospital utilization.
  • No increase in adverse events was observed with same-day discharge.
