Case:
A 50-year-old man sustained a right ankle injury after a 2.5 m fall. Imaging revealed an inverted osteochondral fragment originating from the lateral talar sidewall, a fracture of the lateral process of the talus, and a medial malleolus fracture. Owing to the fragment's size, fixation was performed, resulting in fracture union and favorable recovery.
Conclusion:
This case presents a rare complex ankle fracture pattern with an inverted osteochondral fragment originating from the lateral talar sidewall, likely resulting from a high-energy mechanism. CT or MRI is crucial for identifying this subtle intra-articular pathology and ensuring appropriate surgical planning.
